About us
The Paediatric Outreach Nursing Team (PONT) is a community nursing service providing nursing care at home for children and young people.
The service is for people aged 0-18 years of age who are in full time education and who are registered with a GP within the Kingston ICB.
We work in partnership with patients’ families to ensure the care we deliver is family-centred and tailored to the individual child’s needs.
We will make every effort possible to book appointments in advance at a time and in a setting convenient for you.
This could be at home, in a clinic or at school. Please note, to streamline the service we offer to our families, you will be offered a two-hour slot for your appointment.
We accept referrals from the paediatric units at Kingston Hospital, neonatal intensive care units, GPs and other hospitals.
Please note, we require a minimum of 24 hours’ notice from referral acceptance to appointment time need.
Some of the services we provide
- administration of intravenous antibiotics and medication
- phlebotomy
- administration of medication via intramuscular or subcutaneous injection
- wound care and assessment
- support with enteral feeding
- tracheostomy support
- oxygen at home
- training for schools and nurseries on specific conditions
- end of life care.
PONT is not an emergency service. If your child is unwell, you are advised to seek medical advice from your GP, call NHS 111, or go to your nearest emergency department (A&E).
We are based in an office on level 3, ground floor in the Bernard Meade Wing at Kingston Hospital, next to Sunshine and Dolphin wards.
