About us

The Multiple Sclerosis nurse specialist service supports people with a diagnosis of Multiple Sclerosis (MS) who live in Richmond and have a Richmond GP.

The MS Nurse Specialist is part of the Richmond Community Neuro Rehabilitation team and are based at Richmond Rehabilitation Unit.

What we do

  • Specialist nursing assessment and advice on the management of MS
  • Provide support and information for patients who have recently been diagnosed with MS
  • Ongoing support for patients with MS and their carers via telephone or direct contact
  • Link between community and hospital services     
  • Case management for patients with complex needs     
  • Links to other services supporting people with MS, making referrals when required
  • Education and training for health and social care professionals involved in MS

The MS Nurse Specialist can provide advice over the phone, at clinic or at home.

If you are already known to the service, please contact the MS nurse to express your interest. If you are a new patient, please ask for a referral through your GP or neurologist.

Referrals

This service is available for patients who:

  • are registered with a GP in Richmond
  • have a confirmed diagnosis of MS
  • are experiencing problems associated with MS
  • do not already have a MS nurse specialist

How to refer

Referrals can be made by GP, neurologist or any healthcare professional.

All referrals need to be completed on the Single Point of Access (SPA) referral form and sent to hounslowandrichmond.spa@nhs.net.
